Complete our New Client Application:
Client completes the Medical Statement and Liability Release below and emails back to us.
Have your doctor complete the Doctor Consent Form below and fax back to us.
Give your doctor the Request Letter For Bone Density Scan, which is required for all clients who are not primarily walking after 6 months post injury. Not required for clients under the age of 18 OR if you are primarily walking. This letter may be provided to doctor to request an order for the scan. Scan results must be sent to NextStep.

Once all forms are completed we will schedule your evaluation for our Activity Based Therapy Program
Customized Programs
We customize all of our programs to meet the individual’s needs and goals. Once we have received all required paperwork back from you and your doctor, we will schedule your evaluation which is $300 and is a 2 hour session. After your evaluation, our specialists will recommend the appropriate program for you: Guided Exercise, NMES (Neuromuscular Electrical Stimulation) or Gait Training, and will discuss Membership options.
Physical Therapy and Seating Clinic
We accept insurance for Physical Therapy. Contact us to see if we accept your insurance plan and to schedule your evaluation to get started.

These should be completed by your doctor and MUST be faxed directly to our office at (678) 580-1298 from them. We must receive these before we can schedule your evaluation.
